SHINE Community Services aims to inspire hope and enrich the lives of those whom we serve by providing quality support to older Australians who wish to remain living safely and independently within their own homes, while staying connected to the community for as long as possible. To ensure that goal is reached, SHINE Community Services has a partnership with our four local councils, those councils being the Towns of Cotteslow, Mosman Park, Claremont, and the Shire of Peppermint Grove. 

The aged care system in Australia is being reformed to ensure that it can provide the best possible system for the people of Australia. Here at SHINE Community Services, we are prepared for the anticipated rollover and we are already a recognised provider under the new national administration system. 

The new wellness approach involves assessment, planning and delivery of services and supports that build on the goals and strengths of the individual. This approach encourages you to consider decisions that promote personal independence in daily living tasks, as well as continuing to live safely in your home and in your community. The wellness approach is a philosophy that embodies all activities under the Commonwealth Home Support Programme (CHSP). The wellness approach has a 'doing with' philosophy, and avoids the traditional way of 'doing for'.

Home and Community Care (HACC)


The Home and Community Care (HACC) Programme is a joint Commonwealth, State and Territory funded initiative which began in 1985. The Home and Community Care (HACC) Programme funds basic support services to some older people, people with disabilities and their carers to assist them with continuing to live independently at home. The Home and Community Care Programme (HACC) includes the following: - Support to participate in social activity in a group or one-on-one - Assistance with everyday household tasks - Assistance to enhance nutrition, function, strength, independence and safety - Assistance to support your independence in your personal care activities such as showering and dressing - Assistance to keep up with essential activities such as shopping, banking and maintaining social contacts ​​ More information about the Home and Community Care (HACC) programme can be found here.




Commonwealth Home Support Programme (CHSP)


Commonwealth Home Support Programme (CHSP) is due to take over from the Home and Community Care (HACC) programme as of the 1st July 2018. SHINE Community Services will continue to provide the same excellent care and services under the new programme. The Commonwealth Home Support Programme (CHSP) includes the following: - Support to participate in social activity in a group or one-on-one; - Assistance with everyday household tasks, including minor home and garden maintenance; - Assistance to enhance nutrition, function, strength, independence and safety; - Assistance to support your independence in your personal care activities such as showering and dressing; - Assistance to keep up with essential activities such as shopping, banking and maintaining social contacts; and - Assisting carers in taking a break from their duties in the form of respite care. If you are currently receiving services from Home and Community Care (HACC), then you can expect your services to continue to be the same under the new Commonwealth Home Support Programme (CHSP). Home Care Packages (HCP)


Home Care Packages (HCP) are customised packages that assist you in living independently in your own home. The Australian Government provides a subsidy to an approved home care provider that goes towards a package of care, services and case management. With these packages, you have the flexibility and the final say in who delivers your care. Home Care Packages (HCP) can be used to purchase a wide range of services including, but not limited to: - Clinical care, such as nursing, allied health and physiotherapy for mobility and strength; - Support services, such as help around your home, visiting the doctor and attending social activities; - Personal care, such as assistence with showering, dressing and moving around your home; and - Nutrition, such as assistance with preparing and eating meals, assistance with special dietery requirement for health and wellbeing, and assistance with using utensils. Fee for Service


Our Fee for Service model gives you the flexibility to spend your money as you wish. You will have access to our full range of services and only have to pay for the services you want or the events you wish to attend. Services that are available include, but are not limited to, social club outings, meal services, home maintence, personal care, and domestic services. Additional services can be purchased at any time. To discuss our schedule of fees, please contact us.




Brokerage


Here at SHINE Community Service, our brokerage service model is consistent with our philosophy of providing customers with choice and flexibility in how, when and by whom their care is provided. If you are interested in any of SHINE Community Services, but are currently with another provider, you can request from your provider to allocate some of their servicves to us, so we may better assist you to your satisfaction.
